All applicants for a permit, license or certification issued by the state fire marshal in accordance with the provisions of this code subject to sections 2909.32 to 2909.34 of the Revised Code ("Declaration Regarding Material Assistance/Non-assistance to Terrorist Organization"), shall fully complete the required declaration of material assistance certification form as a part of any application process. Any incomplete certification form shall be considered by the state fire marshal to be an incomplete application for a permit, license, or certification unless the department of public safety reinstates the application. If the applicant provides an affirmative response indicating that the applicant has provided material assistance to a terrorist organization, such application shall be denied by the state fire marshal regardless of the status of the remaining portions of the application unless the application is reinstated by the department of public safety. If any person who receives a permit, license, or certification from the state fire marshal that is subject to sections 2909.32 to 2909.34 of the Revised Code, while such permit, license, or certification is active pursuant to the provisions of this code, takes an action that results in an affirmative response to any question on the certification form, the state fire marshal shall revoke such permit, license or certification.
